As Fall of 2005, the Office of the Registrar will no longer mail grade reports to students. Students may access their grades through "The View". Students who require a hard copy grade report may request it by completing the Final Grade Report Request form and submitting to the Office of the Registrar. Also, request forms are available in the Student Affairs area.

Leave of Absence Request Form
Under special circumstances a student may request a Leave of Absence (LOA) from professional studies. A student may initiate the process by submitting the Leave of Absence request form to the Chair of the SPC. A student requesting a LOA should continue with their academic schedule until he/she receives a response from the SPC Chair.
A student on a LOA may utilize the Learning Resources Center but will not be allowed to check out materials. In addition, the student must relinquish their Student ID for the duration of the LOA. Consequently, a student on LOA is considered a "visitor" and shall receive a visitor badge when they visit the campus.
The College reserves the right to impose a mandatory LOA under circumstances deemed appropriate by the College. The terms of such a LOA will be determined by the College on a case by case basis.

Release of Directory Information

Certain information is classified by the College as Directory Information such as student's full name, local and home address, local and home phone number, professional year, campus mailbox number, and e-mail address. Periodically, ICO receives requests from various groups such as state optometric associations and optometric manufacturers for names of enrolled students. The Office of the Registrar provides a form by which students can authorize the release of directory information to such groups. Under no circumstances, however, does the College release directory information to the general public

Request to Change Lab Section - Requests must be submitted at least two weeks prior to the first day of class. Submit requests to the Scheduling Coordinator to resolve any schedule conflicts first. Otherwise, the request to switch labs will be be denied and the request form will be returned to the student.
